And One More Thing

Americans with a pre-existing condition cannot be denied health insurance coverage.. ever again.

Of course, there's no telling how much you might have to pay for that premium, but if it's employer sponsored, you're golden no matter what.

To be fair.. I thought the mandate was was unconstitutional.. but Roberts tweaked it by changing it to a tax on those that aren't insured. Clever.

Also - apparently in Scalia's ruling, he kept referring to the "Ginsberg dissent" - meaning he had written it up as if he was going to present the opinion of the court.. meaning.. John Roberts flipped on him at the last second and Scalia didn't have time to re-write his opinion.. or have his staff re-write it rather.

It's just the funniest thing in ages.

If Romney wins the election, and the republicans control the house.. and the senate.. all the democrats have to do is prevent a cloture vote in the senate, and repeal cannot happen. Then in 2014 when the law takes full effect, you'll see the Medicare effect.. wherein Americans actually like the program, and that will be that. It will live forever. In order for republicans to get elected, they'll have to support Obama's HCR.

Things That Annoy Me

Working for a contractor to the federal government means you have to do a lot of "training" courses every year.. ethics and compliance and so on. It's basically the same thing over and over, with different pictures of models.

What irritates me is that manager at MegaCorp (I get a new manager basically yearly) sends me multiple emails with increasing persistence as the deadline approaches letting me know that I haven't completed the training yet. The stupid fuckers don't know what deadlines are. In this case, the deadline is July 1. If the deadline was June 1, I would click through the slides by June 1.. but it's not.. it's fucking July 1, and these dickheads send emails anyway.

The stupid fuckers should just tell us the deadline is June 1 if it is, in fact, July 1.. that way I'd get it done by June 1 and they wouldn't have to piss me off with stupid emails.

7 To 3 To 2

Interesting development in my neurological drama. Yesterday, I woke up with the ringing in my ears at about a 3 on the richter scale (0-10). Normally by the end of the day, I'm at about a 7 in each ear. However, yesterday it stayed around the 3-4 range. Again, this morning.. around a 3.. and it's still around a 3 I think by 2pm. I don't have any explanation for the change, other than I've been on prednisone for 2 weeks now, but I did a 10 day course back November without any effect. I'm still sleeping with my head low, which did seem to confirm a positional component to the tinnitus. I think I may take the shim out of the bed and see what happens. When the ringing is at the 7 range, it causes me to feel ill in general.. not just annoyed by the ring. I feel better at level 3, but I don't think completely normal. It's progress I guess. It could snap and go back to the 7 at night thing, or I could wake up tomorrow and have no ring. It's a mystery. I don't like mysteries. /adding I really hate the new blogger interface. I typed this with paragraph breaks, and it appears to display as a wall of text. I'm exponentially pissed off because they don't even give you the option of using the old editor.. which was much better. Fuck.. google

The Human Machine

I think I've suddenly figured out a way to prove the hypothesis about the ringing in my ears, and associated headache I used to have until I started taking meds to prevent the headache.

My problem was that I was trying to be a better neurologist than my neurologist.  I'm not.  I'm an engineer, and I'm a better engineer then he is a neurologist.

Initially, trying to be a neurologist was fruitful. I was able to use my google-fu to align my symptoms to the most likely cause. A slow leak of CSF fluid from somewhere in my head or spinal column. It was like sticking in a bunch of keywords and then sifting through all the results until finding one that matches very closely.  I did.. a study on "second-half-of-the-day-headache as a manifestation of spontaneous cerebral spinal fluid leak".

All the evidence was there.  I had a sudden onset period.. basically one day.  That indicates a physiological change in my nervous system, not a chemical change that would demonstrate gradual onset.  There were multiple symptoms, which slowly resolved to the two main symptoms, headache and tinnitus.  My body must have attempted to repair itself but couldn't complete the job.

There has been a question that I didn't know the answer to because it never matched up to any documentation I could find and it didn't make sense to me.   When thinking about the question as the engineer and not the neurologist, I think I've figured it out.

The question is;  why does my left ear ring louder than my right ear?

Job Tips

One thing that is a concern when you work from home is that you might worry that people think you are slacking off because nobody is looking over your shoulder all the time, as when you're in the office.

The way you fix this is to send work emails at odd hours of the day.  If you wake up at 5am for some reason, go send an email and then go back to bed.  If you're up at 1am for some reason, send a work email and go to bed.

Because of my current medical condition, I often get so sleepy in the afternoon that I can't concentrate on a thing and so I take a few hours nap.  I get emails during that period, but I always check and respond as soon as I can.  When I'm busy, I'll probably get 20-30 emails in a day - not all of which I have to do something with, but it's a lot for a programmer that is supposed to be programming.

Well, I'm not really a "programmer".  I'm a Senior Software Engineer, which sounds more impressive.. but it's basically a better analogy.. most programmers don't do the level of database work that I do to go along with the coding.

Work Until Death

Funny when this shit comes from a gazillionaire CEO of a company that committed massive fraud.

A seemingly off-hand remark made by Robert Benmosche the CEO of AIG has become a bit of a viral online debate, as people of all ages and walks of life weigh in on the idea of bumping up the retirement age to 80. While Benmosche's comments to Bloomberg suggest the age increase is needed to maintain the level of benefits we have been promised and are accustomed to, not everyone is on board with the idea.
They're serious about it.. the very, very wealthy that is.  They know they can retire at any time, and understanding that you can't be rich without most everyone else being poor.. they'll do whatever it takes to gut Social Security and Medicare.

Most people just blow off these sorts of comments, but he's not kidding.  I'm quite sure that's how it's going to go.  The question for me is, will they raise the retirement age for those who are already locked in to a particular age.  For those born before 1960, the age is 65.  For those born 1960 and after, it's 67.  Those two years are not a big deal to me as I expect I'll be able to keep doing what I'm doing for an extra 2 years without much bother, just to get the full retirement benefit.. or maybe there will actually be enough money accrued where I won't care.

The great part about this, of course, is that most companies are not going to want you after you hit 50, and you've still got 30 more years of work ahead of you.  Cat food in the park anyone?

Staying Alive Staying Alive

So ya.. having a spinal tap sucks.  At one point in the needle insertion, the doctor nicked a nerve which sent one of those electric shocks down into my scrotum and into my right testicle.

My testicles are very important to me.. and they are fabulous, just ask anybody who has seen them (which is quite a few people I think).

I haven't written a description of the last visit with the neurologists that lead up to this.  It was unusual, so I'll just describe it now.

I walk into his office with a stack of documentation on what's known as "second-half-of-the-day headache as a manifestation of spontaneous cerebral spinal fluid leak".  You can google that to see an abstract of the study if you want.  So, I explain to Doctor Turban (not his real name, and he doesn't actually wear a turban, at least not in the office) that in our last visit we would attempt to discontinue the nortriptyline and the diazepam.  That was a staged discontinuation.  A few days after the last diazepam, the tinnitus exploded back into my ears with ferocity.  It was so intense that if that were my normal state 24/7, I would have to be institutionalized and put on a thorazine drip.  It was bad.  I got back on the meds and to my current generally okay-but-ears-still-ringing state.

From there, he is basically at a loss for words.. disagrees that I have a CSF leak, and after I ask him how much time must pass before he determines he can't solve the problem and has to consider "long term" solutions.  He says he "doesn't like to think of it that way", and suggests I get a second opinion.  He also says that a radiologist wouldn't consider doing a blood patch (injecting blood into my spinal canal in order to plug any leaks) without there being a positional component to my headache.  In other words, because the headache does not go away when lying down, and comes on when standing up (the CSF leaves the brain in that situation), then the radiologist wouldn't do a blood patch.

Great.  I just handed him documentation that demonstrates that not all CSF leaks have a positional component to the headache, explain that to him, and he says the radiologist still wouldn't do the blood patch. If the leak is slow, the headache may not be positional and an MRI may be normal - both true in my case.  What I'm proposing to the neurologists is a rare condition, but it is a condition that is documented and studied.

Keep in mind, this is a neurologist that has not done a single diagnostic test on me in nearly 6 months of having this condition.

He sensed that I was clearly becoming annoyed and decided that we would try a 1 month course of prednisone, which is a steroid.  I had tried a 10 day course when I saw my PCP the first time after the onset of the symptoms back in November.  They didn't help.

I thought that would be fine.. I'd do the one month course of 'roids and Dr. Turban would, in the meantime, find a neurologist who has an interest in headaches and tinnitus to send me to.  I dropped all attempts to teach him about slow CSF leaks, their symptoms and how they match up pretty damn close to mine, and just agree.. and left.

2 days later, on a Saturday afternoon, Dr. Turban calls me up and explains to me that he doesn't feel like he's given me the "level of care that I deserve".  Duh.  So he orders the lumbar puncture (spinal tap), with opening and closing pressures... which means the radiologist that conducts the test measure my CSF pressure before draining some CSF off to be sent to a lab for tests, and after.

During the course of getting setup to drill into my spine, the radiologists engages me in chit chat where I explain my theory of my problem.  He was an affable guy, and clearly caught a bad case of Teh Gay in his youth.  Guess what he says after he measured the opening pressure;  "the pressure is low".

DUH MOTHER FUCKERS!

The rest of the procedure went fine.. I had to lay on my back for 4 hours before I could go home, which is not comfortable for me.. but whatever.. and now I'm in front of my computer when I'm supposed to be laying in bed.. but whatever, I'm fine.

The low pressure does not prove my theory about the slow CSF leak, but a normal or higher pressure would have dis-proven it.  It'll be interesting to see if the lab results on the fluid show anything abnormal, but at least I have another piece of info.  I want them to do a fucking blood patch on the chance that I'm right.  It would fix everything.  But you know what I found on Wikipedia?  The average duration from symptom onset to diagnoses for a CSF leak is 13 months.  13 fucking months!  I know exactly why that is.

The fix for it, is basically what I had done today, but instead of drawing fluid out, they take some of my blood and inject it into the spinal column.

Anyway.. that's the update.  I'll send Dr. Turban a message tomorrow to let him know the test was performed and he should be getting results soon.

Otherwise, I'm doing okay.

*yawn*

So IQ (the MegaCorp I work for) announced they are laying off something like 27,000 employees.  Apparently all those people weren't doing anything during their work day.  Of course, because I'm subbed to the gov, and I only get a fraction of that money - I'm not going anywhere.

The funny part is that the division of IQ that I work for had a conference call with all us gov grunts, breathlessly announcing that we're not being laid off.  I normally don't call in for that regularly scheduled meeting (in the last 10 years.. heh) but I did today.  As soon as the chief baboon finished 20 minutes of discussion to say "you're not being laid off", she/he then asked for audience participation in the form of questions.

You know a meeting is useless when I haven't attended it in 10 years.  The managerial types, who in reality aren't doing anything useful at all, like to feel all managerial so they have the calls.  It would be more amusing if nobody at all called in, but not everyone is as cool as I am.

MIAMI (AP) — A Miami college student plans to plead guilty in federal court to threatening President Barack Obama on Facebook. 
A plea hearing is set for Wednesday afternoon for 20-year-old Joaquin Amador Serrapio Jr. His attorney says he intends to plead guilty to one count of threatening to kill or harm the president. 
Federal prosecutors say Serrapio posted threats on Facebook in February when Obama was in Miami to give a speech. 
The posts threatened to put a bullet in the president's head and asked if anyone wanted to help in a presidential assassination.

Of course, the amusing bit is the comments to the article, where there are thinly veiled threats of the same sort. I wonder if there is a convenient  web site where you can report threats?  I'd enjoy seeing more right wing freaks going to prison.

I'm still hoping Obama loses as punishment for being a right wing authoritarian freak.. but the seeing the Loon Brigade lose their fucking mind if he wins would be amusing.

....

I still hate the new Blogger interface that they force us to use now.  It takes me twice as long to construct a simple post.  It's irritating as fuck... speaking of which, that reminds me of Windows 8.. what a piece of shit.  That interface is awful.  It's true that every other Windows version is complete shit, but the problem this time is that Microsoft is not likely to abandon the interface.
